Clients are invited to review the Service Agreement form and bring a signed copy to the initial appointment. Alternatively, forms can be filled out at the initial session. If you would like to coordinate your care with other care providers (Psychiatrist, Primary Care Physician, Other Therapist ), please fill out the Consent to Release Information form. Please be mindful of confidentiality if you choose to print these forms.
